Login Now 2019-CES-005: Community & Employment Services Program Specialist (Part-Time) - Baldwin Req Code:2019-CES-005 FT/PT Status:Part Time Salary:$14.18-$18.21 Job Summary: This employee is responsible for training and assisting consumers in the development of vocational, social, recreation, and daily living skills in the community. The employee will assist consumers with job development and coaching in the community, finding housing, participation in community based social and recreational activities, and the development of natural supports. The employee will implement program activities tailored to meet the therapeutic and rehabilitative needs of the consumer. This includes documenting and monitoring the outcomes of consumer objectives, teaching skills associated with the activities of daily living, community integration, physical health, and self-help. The employee must demonstrate excellent public relation skills. He/She will need to build strong relationships with consumers, potential consumers, and community representatives of vocational, housing, recreational, and natural supports resources. Must demonstrate the competency to work directly with developmentally disabled and mentally ill adults and children with severe emotional disturbance (SED). Job Responsibilities: 1. Daily Living Skills Training Teaches skills associated with the activities of daily living including community integration in the realms of housing, social and recreational activities, and independent living activities; physical health maintenance skills, and self-help relative to effective problem-solving, etc. per the consumer’s objectives as identified in their person centered plan. 2. Vocational Training Provides training in the development of vocational skills in the community. In the Sites settings this may include general monitoring, assistance, and work training for consumers, as well as the coordination of work assignments and product deliveries for the consumers who purchase services or products from the work programs. If a consumer is hired into a community based job, this employee may monitor, assist, and provide work training for consumers, as well as work with employers to overcome barriers to consumer success in the job. 3. Activity Implementation Is responsible for implementing a wide range of therapeutic and rehabilitative services for consumers in community settings. Participation in these activities is based on the consumer’s desires and needs as documented in the person-centered plan. 4. Personal Care Based on the needs of the individual consumers in the program, may provide personal care in the form of toileting, feeding, medication management, etc. while involved in activities in the Community and Employment Services program. 5. Develops Natural Supports and Develops Strong Community Relationships and Develops Resources for Employment and Housing Assists in identifying and developing natural supports in the community for the consumer. Develops strong relationships with community representatives of vocational, housing, recreational, and natural supports resources, as well as consumers and potential consumers of services staffed by consumers or products made by consumers. Responsible for finding employment and/or housing for the consumer, as appropriate per the consumer’s person centered plan. 6. Support Services Assists with identifying coping strategies for successfully caring for or living with a person with a disability and/or ways to support the person with the disability in day-to-day problem solving and connection into the community. This support may be provided one-on-one or in a group setting. Job Qualifications: 1. At minimum of a High School Diploma or G.E.D is required. 2. Experience in working with MI, DD, and SED consumers is desired. 3. The employee is required to become a certified Employment Training Specialist at the employer’s expense within 12 months of being in this position. 4. Must possess a valid driver’s license and provide own transportation to and from meetings and activities at varying work locations including all agency locations in Lake, Mason, and Oceana Counties. 5. Must be certified in First Aid and CPR. 6. Lived experiences with mental illness/developmental disabilities/substance use disorders are valued.
